The sleeve is gradually eased up the limb so that the material (and therefore the pressure) is evenly distributed. Make sure … • … that there are no creases or wrinkles since these will chafe the skin and act like an elastic band, causing fluid to build up below. • … that the sleeve is not too tight. It should feel firm and supportive, but should not be painful or make the fingers turn purple or blue. • … that the sleeve is not too loose or baggy, especially nearer the hand, otherwise it will not do its job. If the sleeve is loose at the hand but fits the rest of the arm well, you will need to insert a pad between the sleeve and the back of the hand. A small piece of foam rubber is ideal; cotton-wool pads do not work well. An additional mitten may be applied beneath the combined armsleeve. The tops of the sleeves should never be rolled down as this can act like an elastic band, causing fluid to build up below. If the sleeves seem too long just ease excess material evenly down the limb until the length is right. The use of talcum powder on the skin will assist with fitting the sleeves, particularly after the limb has been washed. Some patients will experience mild discomfort during the first 24 hours. However, this is almost always transient. Skin hygiene is important. The use of creams and lotions is not contraindicated as the fabric of the products is resistant. Armsleeves and mittens may be removed when retiring to bed at night unless the doctor/therapist has advised otherwise. Patients should advise their doctor/therapist if during the wearing of these garments they experience: • Pain • Swelling of the fingers • Numbness in part of the limb or • Colour change in hand or fingers 49 1. Position the Butler with the semicircular opening facing away from your body. Insert the armsleeve. 2. Now gather up the upper border of the armsleeve over the inner frame of the Butler. Then push the arm - sleeve down evenly over the frame using both hands. 3. Check that the armsleeve fits properly. The knitting marks at the elbow should be facing the wearer. 4. Now push your hand into the opening. Hold the armsleeve carefully on the Butler with your other hand as you do so. 5. Place the Butler on the edge of the table and push your arm into it slowly. 6. 71 Simply put on compression stocking 1 First stand the medi Butler on a firm surface with the broader side of the base facing you, and the opening in the semicircular frame facing forwards. Lay the compression stocking in the stirrup with the toe of the stocking facing forwards. Turn the upper border of the stocking inside out and over the frame. 2. Slide the stocking down over the stirrup until the heel appears at the edge of the stirrup. 3. Now place the medi Butler on the floor. Slip your foot, toes first, into the toe of the stocking. It is recommended that you lean against something firm (table, wall) for stabilisation. 4. Now step into the rigid stand. Please check that the toe and the heel are correctly positioned. 5. By alternately pulling and releasing the two handles you can now slide the stocking up towards your knee . 6. When the stocking is just below the knee tilt the medi butler slightly backwards and remove it from the stocking. 7. To finish, smooth the compression stocking upwards evenly. Please make sure you ease the stocking up bit by bit without gathering it up or pulling on the top.
